New Patient Registration

To register you can complete and sign a registration form and online medical questionnaire available from reception. Alternatively, you may download, print and complete the Registration Form and online medical questionnaire at home and bring them to surgery with personal identification to register. If you would also like to have online access please complete the Online Access Form. When completing a registration form you must complete all relevant parts of the form:

• Full address with postcode
• Your date of birth
• Include your NHS number, if you know it
• A previous address and GP must be provided unless this is your first time registering
• Immigrants must provide a date of entry into the UK
• All babies must be registered with an NHS number on the form
• Service personnel - recently discharged, and people returning to the UK from abroad must provide their previous address prior to joining the Services or leaving the UK

We ask patients to fill out the medical questionnaire because it can take a considerable time for us to receive your medical records.

Patients register with the Practice rather than a particular doctor. If you do have a preference of which GP you see, this can be recorded when you register. You can also ask to see a particular doctor when making an appointment. This will be accommodated where possible, but may mean waiting longer for an appointment.

You can choose to make an appointment for a New Patient Health Check with one of the Nurses or alternatively a Doctor (only if your health needs are complicated) and also to complete a new patient questionnaire.

• As part of the registration process, it is NHS policy to ask for a patient's ethnicity
• Any new patients who have recently arrived in the UK will be asked for proof of residency
• All patients aged 16 or above will be asked for some form of personal identification (including photographic ID eg passport or driving licence) which you must present in person

We ask for identification for two reasons:

• To prevent fraudulent registrations.
• To reduce the duplication of patient's records across the NHS.

The Practice Area

We accept Registrations from any address within the Practice Area. This is defined as the area bounded by the A3M to the East, Butser Hill to the North and a line encompassing Day Lane (Lovedean) Crouch Lane, Fiveheads Road and Lith Avenue to the South. To the East the border is defined by The Bat and Ball PH, Hyden Farm Lane, Old Mill Lane and Broadway Lane to the junction with Day Lane.

Change of Personal Details

If you move address, change your name or telephone number, please notify the surgery as soon as is convenient.

It is important to inform us of these changes in case we need to telephone or write to you.

We also recommend that you also inform any hospital or clinic that you are receiving treatment from or have been referred to.

A change of address may mean that you are outside of our Practice Area and may need to register with another doctor.

We normally ask for proof of a change in name (i.e. marriage certificate) and you will always have to provide proof if there is a discrepancy over a date of birth.
